Micro-TESE
Microsurgical testicular sperm extraction (microTESE) is a surgical procedure used to retrieve sperm from inside a man’s testes (organs that produce sperm)
It is used for men with non-obstructive azoospermia, which is when a man cannot produce enough sperm to have a detectable amount in his semen, a common cause of male infertility.

Why Micro TESE is better than a biopsy?
- Micro-TESE is usually reserved for non-obstructive azoopermia, for those who have already had a fine needle aspiration or biopsy without recovering sperm. However, there is scientific evidence to show that for men with non-obstructive azoospermia, fine needle aspiration or biopsy before micro TESE may be not recommended.
- The success rate of Micro testicular sperm extraction (Micro-TESE) is quite favorable for men. In fact, doctors can find sperm about 60 % of the time during micro TESE procedures.

How is Micro-TESE Performed?
Micro TESE procedure is performed in the following cases-
- Sperm and testosterone (male hormone) are produced in the 2 testicles of the scrotum.
- Sperm leave the testicles through a rolled tube called the epididymis. There they remain until they are ready to be used.
- Each epididymis is attached to the prostate by a tube called the vas deferens. This duct runs from the scrotum to the groin, then the pelvis and behind the bladder.
- There, each vas deferens joins the seminal vesicle and forms the ejaculatory duct.
- For ejaculation, sperm pass through the ejaculatory ducts to mix with fluid from the seminal vesicles, prostate, and other glands to form semen.
- Semen travels through the urethra and exits through the end of the penis
Micro-TESE procedure is performed when the goal is pregnancy but is not possible without help. It is for men who have little or no sperm in semen, or men who are not able to ejaculate.
In these cases, sperm can be collected from other parts of the reproductive tract. To obtain better pregnancy rates, Micro TESE process is used with in IVF and intracytoplasmic sperm injection (ICSI).
Recovery After Micro-TESE Procedure?
- To manage the swelling, using ice pack is usually recommended for the next 24 hours from the procedure.
- A combination of ibuprofen and acetaminophen is also recommended to lower discomfort and uneasiness.
- Scrotal swelling is to be expected with this process. The swelling may take some weeks to go down completely.
- Some other medicines may also be prescribed as an additional pain reliever.
- It’s highly recommended to avoid sex, masturbation, and vigorous activity for at least 8-10 days after this surgical process, to allow your testicles to recover completely.

Andrology, Urology & Micro Testicular Sperm Extraction (Micro TESE) FAQs-
The Micro-TESE surgery is carried out in the outpatient clinic. Its duration is usually between 1.5 – 3 hours depending on the intraoperative findings and the patient can leave the clinic 4 hours post the surgery. Please note that you are not allowed to actively participate in traffic within 24 hours of anaesthesia.
We carry out the surgery under general anaesthesia. You will have no pain during the surgery. After the surgery, an uncomfortable feeling and pressure in the operating area, especially when walking or during physical exertion, are possible. Severe pain is not expected after this surgery. The symptoms can be alleviated by resting, cooling the scrotum and pain killers (occasionally).
After the surgery, you will be need rest for a week. You should avoid heavy physical exertion or sports activities within 4 weeks from the surgery. The patient is expected to visit the clinic 1-2 days after the surgery for check-up.
